Listed below are the most common types of request forms used by the PTO. Click the button for access to an electronic form or PDF form. Staff, parents, and students can all fill out and submit these forms. (Due to COVID-19, no hard copy form requests will be accepted. All requests must be submitted digitally. You may still print and fill out the form, it should then be emailed to us though-as opposed to dropping it off at school.)
Completing a form:
1. Click the appropriate form button below. Fill in blanks as prompted.
2. For an e-form, submit as instructed on the form. For PDF forms, please attach completed form to an email and send it to firstname.lastname@example.org. Receipt images for all form types can be emailed to email@example.com.
3. PTO Treasurer will receive an email with your form submission. Depending on the type of follow-up needed from the Treasurer, please allow up to two weeks for processing. Reimbursements will be processed through VENMO or our bank electronically.
Note: We do our budgeting and plan our fundraisers based on the prior year’s expenses. If our volunteers do not submit receipts for reimbursement, we could under-report our expenses resulting in budgeting problems for future years. We ask that our volunteers do not “eat” expenses. If you wish to donate items you have purchased, please contact the Treasurer to ensure that the donation is correctly accounted for. We will gladly accept your donation!
